Policies Patients are seen in our office by appointment. We make every effort to see our established patients with urgent needs within 24 hours, however we also try to accommodate our new patients as soon as possible.
Patient Instructions: HEALTH HISTORY: Please bring your complete health history including your past and current medical and surgical history as well as family history.
ALLERGIES: Please let us know your drug and food allergies and keep us updated with this information on an ongoing basis.
MEDICATION LIST: Please bring a complete list of all of the prescription and non- prescription (over-the-counter) medications that you are currently taking. Be sure to include the exact dosages. You may also bring in all the medicine bottles/packaging to show the doctor. This will help us in providing better, more timely care for you. Many medicines interact with each other and the doctor will be better able to determine an appropriate treatment plan for you with this information.
INSURANCE CARD: Always bring your current insurance card with you. This will expedite your paperwork and enable us to bill your insurance accurately. We also encourage you to find out ahead of time what your insurance covers. This will help you in making your treatment decisions. Co-pays are due at the time of services.
PRESCRIPTION RENEWAL - PROVIDE PHARMACY PHONE NUMBER: When calling us to renew a prescription, please give us the phone number of the pharmacy your want us to call for your prescription. (Prescriptions can be delayed when we have to track down which pharmacy to call.)
PRESCRIPTION RENEWAL - PLAN 3 DAYS AHEAD Please plan ahead and call to renew your prescriptions AT LEAST 3 days prior to running out of medication.
